WHAT INFORMATION DO I NEED
TO PROVIDE WITH MY CLAIM?
When submitting a claim for compensation you must
provide the following information (see paragraph 2.4.1):
✓ ● Your name and address.
✓ ● The name of the ship involved in the incident.

✓ ● The date, place and specific details of the incident,
if you know them, unless the 1992 Fund is already
aware of that information.
✓ ● The type of pollution damage caused by the incident.
✓ ● The amount of compensation you are claiming.
You may need to provide additional information depending
on the type of claim you have (see Section 3).
You may find it helpful to read the example Claim Form
available on the IOPC Funds website. In the event of a
major incident, you will be able to download an incident
specific-claims form that you should complete and
return to the IOPC Funds/shipowner’s insurer.
